2 Section A - bottom right - a box for registered body use only a1-a3; a14; a21; a23; a25 You complete this as the ID check is done. There is also a box on the opposite page that you must complete

5 Box 64 to 68 Q64 and & Q65 we beleive will be Yes and No respectively. Current advise is to leave blank and flag to Janet till the BAB advises on a stick-it
Q66 No as Child Protection policy does not allow this
Q67 Should not be a problem to decide
Q68 Free of charge volunteer see the TMG Guidance - if no money paid then this will be Yes
6 Who signs at Box 72 You do as verifier / ID Checker
7 Who pays and how much Clubs pay intially and claim back from the Aikikai. £17.50 is paid provided the application is for a volunteer. Made out to TMG CRB
8 Where is the form sent
    To TMG in Nottinghamshire the address is on the front of the form. Before sealing the form. Before sealing the envelope....
    - is the completed form in it;
    - any continuation sheet;
    - the ID form;
    - and cheque
9 Should I enclose my name and address in case they need to come back to me. This seems a good idea
